Injury Waiver

In consideration of acceptance of this registration by The Curling Club of Collingwood ("the Club"), I HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE the Club and each of its members, officers and employees FROM ANY AND ALL claims, demands, damages, costs, expenses, actions and cause of action, whether in law or equity in respect of death, injury, loss or damage to myself or property, arising or to arise by reason of my participation in the Club, that has not been contributed to or occasioned by any negligent act, by omission or commission, of any of the foresaid.

The Curling Club of Collingwood strongly recommends that its members wear protective head gear.

Membership Selection Instructions

To register for on-ice leagues or programs:

Under Membership Selections (with leagues) click the General Curling button and select your membership fee level from the drop down list.
Click the box(es) next to your league selections.
Enter any information required in the Notes for League Manager text box. This is where you can indicate your prefered position(s) for each league and if you are away for any draws.
If entering in a team league you must already have a team fielded. The team name is your skip's last name.
During The Priority Registration Period, indicate up to two priority leagues in the "Other Information" area at the bottom of the registration form.They will only be respected for registrations received during the Priority Periods
Returning member Priority Period is June 23rd to July 6th
New member Priority Period is August 1st to August 15th

To register for Adult Social: Scroll down to the bottom of the form to Membership Selections (no League). Click the button next to Adult Social Member.

Registration Accuracy Confirmation

I hereby confirm that the information I have entered on this form is correct and true. Once your registration is competed you will be taken to the payment screen where you can make your payment online using Visa or Mastercard after completing your registration. Make sure to complete the online payment process BEFORE selecting the print completed registration form option. Failure to do so will close out the online payment option. If this happens, continue the registration process to completion. You will have the option of going into your Profile later and completing payment.

Registrations is only considered complete once payment is received.

Registration Refund Policy:

Full refunds will be issued if registration is withdrawn before September 15, thereafter refunds will only be issued for medical reasons.

Refunds will only be issued for medical reasons, and the refund will be prorated, as follows, to the date the refund is requested. A member may receive 75% of their curling registration payment until November 30, 50 % until December 31 and 25% until January 31 of the current curling season. The Club will not provide membership refunds after January 31 of the current curling season. The Capital Fund component of the registration fee is not refundable.

If the Member chooses to remain on the roster, they can elect to convert their current membership to a Social Member. If they elect this option, the applicable refund less the Social Membership fee will be refunded.
